What causes shoulder pain?

Shoulder pain can stem from a variety of causes, often related to injury, overuse or underlying medical conditions. One of the most common causes is rotator cuff injuries, which involve tears or inflammation of the muscles and tendons that stabilise the shoulder joint. These injuries can occur suddenly due to trauma or develop gradually from repetitive overhead activities, leading to pain, weakness and limited range of motion.

Another frequent cause of shoulder pain is arthritis, particularly osteoarthritis or rheumatoid arthritis. Arthritis causes the cartilage that cushions the shoulder joint to wear down or become inflamed, resulting in stiffness, swelling, and chronic discomfort. Frozen shoulder (adhesive capsulitis) is another condition that causes pain and stiffness, characterised by the gradual loss of shoulder mobility due to thickening and tightening of the joint capsule.

Types of shoulder pain

We can diagnose the different types of shoulder pain based on origin, nature and pain duration.

Acute Pain

Acute pain is a sudden, sharp pain caused by injuries like rotator cuff tears, dislocations or fractures. It typically occurs after trauma or intense physical activity and may be accompanied by swelling and limited movement.

Neuropathic Pain

This type of pain is caused by nerve irritation or damage and is often described as burning, tingling or numbness. Conditions like cervical radiculopathy or nerve impingement can lead to neuropathic shoulder pain.

Chronic Pain

This is a persistent, dull ache that lasts for weeks or months and is commonly associated with conditions such as osteoarthritis, frozen shoulder or tendinitis. Chronic pain often worsens gradually and can lead to stiffness and reduced range of motion.

Inflammatory Pain

Pain caused by inflammation of the shoulder’s soft tissues, such as bursitis or tendonitis, usually presents as tenderness, swelling and discomfort. Symptoms worsen with activity.

Referred Pain

Sometimes, pain felt in the shoulder actually originates from other parts of the body, such as the neck, heart or diaphragm. For example, nerve compression in the spine can cause pain that radiates to the shoulder. Some heart conditions may also cause shoulder discomfort.

Treatments

Ultrasound-guided shoulder injections with steroid, platelet rich plasma – The use of ultrasound guidance ensures precise needle placement into the targeted structure (joint, tendon, or bursa), improving accuracy and safety.
